What it is, How it works

Hair analysis is recognized as a robust method for identifying drug and alcohol consumption. The hair serves as a chronicled record of alcohol and other substances by encapsulating biomarkers within the strands as they grow. When procured close to the scalp, it can offer up to a three-month detection period for alcohol and drugs. Hair is uncomplicated to gather, moderately resistant to tampering, and straightforward to dispatch.

A 1.5-inch section of roughly 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and verification. For EtG, add-ons, and/or assessments exceeding 10 panels, 150mg of the sample is advisable. Utilizing a jeweler’s scale to weigh the specimen is suggested. If there is no head hair, a similar volume of body hair can be collected. The term head hair exclusively pertains to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of incorporating a sample into a laboratory’s system. It involves confirming that the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid assessment for abusive drug presence. Although Screening offers a cost-efficient approach to exclude drug use in the majority of samples, a positive outcome necessitates confirmation for courtroom validity. Samples displaying presumptive positivity in Screening require secondary verification.

If a sample shows presumptive positivity during the Screening phase, additional hair is extracted from the original sample for the Extraction process. During this phase, drugs are isolated from the hair at significantly lower concentrations than in other approaches (such as urine or oral fluid), which makes hair drug analysis the most challenging method to execute.

Confirmation of any positive screening result is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive status are cleansed before confirmation if necessary. The entire laboratory sequence from Accessioning to Confirmation undergoes review according to the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
