What it is, How it works

A segment measuring 1.5 inches of approximately 200 hair strands (roughly the width of a #2 pencil) closest to the scalp yields 100mg, which is the optimal sample for both screening and confirmation. For EtG, additions, and/or tests exceeding 10 panels, a specimen mass of 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair isn’t present, an equivalent quantity of body hair may be used. When mentioning head hair, it strictly refers to scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the sample’s initial entrance processing into the laboratory’s framework. This includes ensuring the sample was properly sealed and conveyed, generating a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not furnished by an electronic chain of custody system.

Screening conducts an initial swift assessment for drug misuse. While Screening is a cost-effective method for discounting drug usage on most samples, a positive screen needs verification to be court admissible. Any samples yielding a presumptive positive during Screening call for a secondary confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the original sample and readied for Extraction. At this juncture, drugs are derived from hair at a significantly reduced concentration than other methodologies (e.g. urine or oral fluid), making hair drug screening the most challenging approach to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation when necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
