What it is, How it works

When collected near the scalp, hair can offer up to nearly a 3-month window for detecting alcohol and other substances. Collecting hair is straightforward, relatively resistant to tampering, and easy to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (approximately the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal sample for testing and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg specimen is suggested. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair is inaccessible, an equivalent quantity of body hair can be collected. When mentioning head hair, it pertains solely to scalp hair, whereas body hair refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial examination of a sample within a laboratory's system, involving the verification of the sample's proper sealing and shipping, the assignment of a random LAN (Laboratory Accessioning Number), and the completion of any extra data entry not available through an electronic chain of custody framework.

Screening performs a preliminary rapid assessment for substances of abuse. While Screening offers a cost-efficient method to negate drug usage in most samples, a positive screen necessitates confirmation to be court-admissible. Presumptive positive samples in Screening require a secondary confirmation.

Should a sample indicate presumptive positivity during Screening, more hair is extracted from the initial specimen for Extraction. In this process, drugs are derived from hair at much reduced concentrations than alternative methodologies (e.g., urine or oral fluid), which is why hair drug screening presents the greatest challenge.

Confirmation for any positive screen result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Prior to confirmation, all presumptive positive samples are cleansed as required. The comprehensive lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
