If scalp hair is not present, an equivalent volume of body hair is acceptable. "Head hair" refers solely to scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

There are four key stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory phase, where a sample is entered into a lab's system. It includes checking that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing further data entry not provided by an electronic chain of custody tool.

Screening involves a primary rapid test for drugs. Though it's cost-efficient for most samples, a positive result from Screening requires verification for legal purposes. Any samples showing potential positivity in Screening need a secondary confirmation.

If Screening suggests a positive result, more hair is taken from the original sample for Extraction. This process involves extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to execute.

Confirmation of any positive result in Screening is completed with GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing potential positivity are pre-cleansed before confirmation, as necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
