What it is, How it works

As a robust method for identifying drug and alcohol consumption, hair testing has gained recognition. The fibers of a growing hair strand capture biomarkers, providing a detailed timeline of substance usage. When hair is collected near the scalp, it offers a detection window for up to approximately three months for both alcohol and drugs. Hair collection is straightforward, relatively tamper-resistant, and easy to transport.

An optimal screening and confirmation sample consists of a 1.5-inch piece of about 200 hair strands (similar in size to a #2 pencil) nearest to the scalp, weighing 100mg. For EtG, additional tests, and/or over 10-panel tests, a 150mg sample is advised. Using a jeweler’s scale to weigh the sample is suggested. If scalp hair isn't available, a comparable quantity of body hair can be collected. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other types (like facial or axillary hair).

Process Overview

The four crucial steps in processing a drug test in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory’s system. This step includes checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available via an electronic chain of custody.

Positive results from screening are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as necessary. The entire lab process from Accessioning to Confirmation adheres to both CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
