What it is, How it works

Hair captures a prolonged record of alcohol and other compounds by embedding biomarkers within the strands' fibers. When sourced near the scalp, hair offers a potential detection span of three months for alcohol and drugs. This method is straightforward to gather, less susceptible to tampering, and conveniently shippable.

A sample size of 1.5 inches comprising around 200 hair strands (equivalent to a #2 pencil's diameter) near the scalp yields 100mg of hair, ideal for assessments and verification. For EtG, add-ons, and tests surpassing 10 panels, 150mg of the sample is advisable. Use a jeweler’s scale to weigh the sample. In the absence of scalp hair, a comparable quantity of body hair can be secured. "Head hair" specifically denotes scalp hair, while "body hair" refers to other types, such as facial and axillary.

Process Overview

The laboratory analysis of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with entering a sample into a lab's system. This entails confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry omitted by an electronic chain of custody system.

Screening functions as a preliminary assessment for substances. While it is a cost-efficient means to dismiss drug use in numerous samples, a positive screening requires confirmation to be legally recognized. Samples testing presumptively positive in Screening must undergo further confirmation.

For samples testing presumptively positive during Screening, additional hair is extracted from the original sample for Extraction. Drugs are sourced from hair at significantly reduced concentrations than in other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Any positive screening outcomes are verified using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to verification as necessary. The complete laboratory process, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
