A sample measuring 1.5 inches with roughly 200 hair strands (approximately the thickness of a #2 pencil) near the scalp provides 100mg of hair, which is optimal for screening and confirming. For EtG, additional criteria, and/or tests exceeding 10 panels, a specimen of 150mg is advisable. We suggest using a jeweler’s scale to weigh the sample. In cases where scalp hair is not available, an equivalent quantity of body hair is acceptable. When we mention head hair, we specifically mean scalp hair, while body hair refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

In the Accessioning phase, the sample is initially processed into the laboratory’s system. This process involves confirming the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any needed data entry not covered by an electronic chain of custody system.

Screening serves as a preliminary check for substances of abuse. Although Screening is an economical method to dismiss drug use for most samples, any positive result must undergo confirmation to be permissible in legal proceedings. Any presumably positive Screening samples need a subsequent confirmation.

If a sample is presumed positive during the Screening phase, additional hair is extracted from the original sample and readied for Extraction. In this phase, drugs are withdrawn from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to conduct.

GC/MS, GC/MS/MS, or LC/MS/MS techniques confirm any positive screening outcomes. All samples with presumptive positives are cleansed before confirmation as necessary. The full laboratory procedure, from Accessioning to Confirmation, is conducted under the CAP (College of American Pathologists) Hair designation and complies with the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
