What it is, How it works

A 1.5-inch segment of approximately 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for analysis and verification. For EtG, additional tests, or those exceeding 10 panels, a specimen of 150mg is advised. Use a jeweler’s scale to measure the sample. If scalp hair isn’t available, a comparable amount of body hair can be taken. By head hair, we mean scalp hair strictly. Body hair refers to all other hair varieties (e.g., facial, axillary).

Process Overview

The key phases in lab processing of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample within a lab's system, confirming correct sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and completing any other required data entry not captured by an electronic custody system.

Screening conducts an initial assessment for drugs of abuse. While cost-effective for ruling out drug use in most samples, a positive screening must be verified for legal admissibility. Any samples indicating a presumptive positive in Screening necessitate further confirmation.

Should Screening suggest a presumptive positive, additional hair is extracted from the original specimen and readied for Extraction, where the drugs are withdrawn from the hair at far lesser concentrations compared to methodologies like urine or oral fluid, thus making hair drug screening the most challenging technique.

Confirmation of any positive screening result is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed as necessary prior to confirmation. The complete lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
