When cut near the scalp, it can reveal up to a three-month period of drug and alcohol use. Collection is straightforward, tampering is difficult, and it is easy to transport.

A sample of 1.5 inches comprising roughly 200 hair strands (comparable to a #2 pencil) taken closest to the scalp yields 100mg of hair, which is the optimal quantity for analysis and verification. For EtG, additional tests, and examinations exceeding 10 panels, a 150mg sample is preferred. It's advisable to use a jeweler’s scale to weigh the specimen. If scalp hair is not accessible, a comparable amount of body hair may be utilized. References to head hair specifically mean scalp hair, while body hair encompasses other types (facial, axillary, etc.).

Process Overview

There are four primary steps in the laboratory procedure for a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a lab's system. This step includes checking that the sample is sealed and transported appropriately, assigning it a random LAN (Laboratory Accessioning Number), and completing supplementary data entry absent in an electronic chain of custody framework.

Screening involves a preliminary rapid test for drug residues. Although Screening is a cost-efficient method to eliminate drug usage for most samples, a court-admissible result necessitates a positive screen to be confirmed. Samples that are potentially positive in Screening demand an additional confirmation process.

In instances where a sample tests potentially positive during Screening, additional hair is extracted from the original sample for the Extraction process. At this point, drugs are extracted from the hair at significantly lower concentrations than in other testing methods (such as urine or oral fluid), which explains why hair drug testing is the most challenging method.

Any positive results from Screening are verified using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Before confirmation, any potential positive samples undergo a cleansing process as required. The entire laboratory workflow from Accessioning to Confirmation is scrutinized under the guidance of the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
