A sample consisting of 1.5 inches of about 200 hair strands (roughly the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is considered an optimal specimen for screening and confirmation. For EtG, supplementary, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is not available, an equivalent quantity of body hair may be used. By head hair, we mean scalp hair only. Body hair includes all other types such as facial and axillary hair.

Process Overview

The drug test result processing in a laboratory involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial sample processing into a laboratory system. This process verifies that the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any secondary data entry not covered by an electronic chain of custody system.

Screening conducts a preliminary, rapid check for drugs of abuse. Although Screening is a cost-efficient way to exclude drug usage in most samples, a positive screen necessitates confirmation for court admissibility. Samples that show presumptive positive results in Screening require a secondary confirmation.

For samples with presumptive positive outcomes in Screening, additional hair is extracted from the initial specimen for the Extraction phase. Here, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire laboratory process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
