A sample measuring 1.5 inches, consisting of about 200 hair strands (comparable in size to a #2 pencil) closest to the scalp, will yield 100mg of hair, which is the preferred amount for screening and verification. For EtG, additional tests, and tests beyond 10 panels, a specimen amount of 150mg is advisable. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair isn't available, a comparable amount of body hair can be obtained. When mentioning head hair, it exclusively refers to scalp hair. Body hair pertains to all other hair varieties (like facial or axillary hair).

Process Overview

The key stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample within a lab's framework. This process involves ensuring that the sample was properly sealed and dispatched, assigning it a random LAN (Laboratory Accessioning Number), and completing any essential data entry not covered by an electronic chain of custody protocol.

Screening serves as an initial rapid test for drugs of abuse. Although Screening is an economical approach to discount drug usage for most samples, any positive screen requires confirmation to be valid in legal proceedings. Samples that show a presumptive positive outcome in Screening require a further confirmation step.

In cases where the Screening phase yields a presumptive positive, additional hair is sourced from the initial sample and prepared for Extraction. In Extraction, drugs are isolated from hair at concentrations much lower than in other methods (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive Screening result is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as necessary. The entire lab process, from Accessioning to Confirmation, is assessed under both CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
