What it is, How it works

Hair testing has emerged as a highly effective method for detecting drug and alcohol consumption. Hair can store a long-term record of alcohol and drug use by encapsulating biomarkers within the fibers of the growing hair. When collected near the scalp, hair offers an approximately three-month detection timeframe for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and convenient to transport.

A 1.5-inch segment consisting of about 200 hair strands (comparable to a #2 pencil's thickness) nearest to the scalp yields 100mg of hair, which is an ideal amount for both screening and confirmation. For EtG tests, additional testing, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's best to weigh the sample on a jeweler's scale. If scalp hair isn't available, a similar amount of body hair can be used. When discussing head hair, we are specifically referencing scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory's system. This step includes verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent in an electronic chain of custody framework.

Screening consists of an initial rapid assessment for potential drug abuse. Although Screening is an economical method to detect drug usage in most samples, confirmation is necessary for a positive screen to be legally acceptable. Any samples that are preliminarily positive in Screening require a follow-up confirmation.

If Screening suggests a positive, additional hair is retrieved from the original specimen and readied for Extraction. Here, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Positive screening results are validated using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as required. The entire laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
