A 1.5-inch segment of around 200 hair strands (equivalent to the thickness of a #2 pencil) nearest to the scalp offers 100mg of hair, which is the optimal sample size for analysis and verification. For EtG, additional tests, and/or screenings exceeding 10 panels, a specimen of 150mg is advised. We suggest measuring the specimen using a jeweler's scale. If scalp hair is not available, an equivalent quantity of body hair can be used. By head hair, we mean scalp hair specifically. Body hair encompasses all other hair types such as facial and axillary hair.

Process Overview

The four key phases in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial intake of a sample into a laboratory's workflow. This step confirms that the sample was sealed and transported securely, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data entry that is not covered by an electronic chain of custody system.

Screening incorporates a preliminary assessment for substances of abuse. Although Screening is a cost-efficient method to exclude drug usage in most samples, a positive finding must be verified to be accepted legally. Samples that show presumptive positivity in Screening require further confirmation.

When a specimen is presumptively positive at the Screening phase, additional hair from the original sample is extracted and prepared for the Extraction phase. Here, substances are isolated from hair at concentrations much lower than those in other testing methods (e.g., urine or oral fluid), making hair drug analysis one of the most challenging methodologies.

Confirmation of any positive finding in Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity are cleansed before confirmation where necessary. The full laboratory procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
