For EtG, additional panels, and/or tests exceeding 10 panels, a specimen of 150mg is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn't present, an equivalent amount of body hair can be taken. When mentioning head hair, we only refer to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing into a laboratory’s system. This stage ensures that the sample is properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data entry not covered by an electronic chain of custody system.

Screening offers a preliminary assessment for drugs of abuse. While cost-effective for eliminating drug use in most samples, a positive screen must be verified to be court-admissible. Samples that are presumptively positive in Screening require a secondary affirmation.

If Screenings indicate a presumptive positive, more hair is extracted from the original sample and prepared for Extraction. This stage involves extracting drugs from hair at concentrations lower than in other methodologies (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of positive screening results uses GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positives are washed prior to confirmation as needed. The full laboratory process, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair specification and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
