A sample measuring 1.5 inches and containing around 200 hair strands (comparable to a #2 pencil's thickness) from near the scalp yields 100mg, the ideal quantity for testing. For EtG, additional tests, or those beyond 10 panels, a 150mg sample is advisable. We suggest using a jeweler's scale for measuring. If scalp hair is absent, collecting an equivalent bulk of body hair suffices. "Head hair" denotes scalp strands; in contrast, "body hair" encompasses facial, axillary, and other types.

Process Overview

The sequence of steps in lab processing for a drug test result comprises Accessioning, Screening, Extraction, and Confirmation.

Accessioning introduces the sample into a lab's system. This phase checks the sample's proper sealing and shipping, assigns a LAN (Laboratory Accessioning Number), and finalizes any pending data entry not captured by an electronic chain of custody system.

Screening provides an initial, rapid assessment for drugs. While cost-efficient for eliminating drug use in most samples, a positive screening requires confirmation for legal acceptance. All presumed positive results during Screening need further validation.

When a presumed positive arises from Screening, additional hair is collected from the primary sample for Extraction. In this phase, substances are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluids), increasing the complexity of hair drug testing.

Confirmation of any positive screening is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumed positives are washed if necessary. Each lab process, from Accessioning to Confirmation, undergoes review by CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
