For EtG, additional components, and/or assessments exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen with a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may substitute. When discussing head hair, it references only scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails initially processing a sample into a laboratory's system. It consists of confirming that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody system.

Screening implies an initial swift assessment for drugs of abuse. While Screening serves as a cost-effective method to rule out drug use in most samples, a positive screen demands confirmation for court admissibility. Any samples showing presumptive positivity in Screening indeed require a secondary confirmation.

If a sample appears presumptively positive during the Screening phase, additional hair is sourced from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), presenting hair drug screening as the most challenging methodology to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
