For EtG, supplemental tests, and/or panels exceeding 10, 150mg of specimen is advised. We suggest weighing the specimen on a jeweler’s scale. If scalp hair is not obtainable, an equivalent amount of body hair can be collected. "Head hair" specifically refers to scalp hair only. "Body hair" encompasses all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of drug tests encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially integrated into the laboratory system. This phase verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data not covered by an electronic chain of custody system.

Screening serves as a preliminary assessment for drug abuse. While it is an economical approach to potentially rule out drug use, initially positive results must be confirmed for court admissibility. Presumptively positive samples from Screening require further confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the original specimen for Extraction. Confirmation of positive screenings is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are washed before confirmation as required. The entire laboratory process, from Accessioning to Confirmation, is reviewed under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
