Collecting a 1.5-inch segment of about 200 hairs (comparable to the girth of a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for both initial and confirmatory tests. For EtG, additional tests, and/or panels exceeding 10, a specimen quantity of 150mg is advisable. It is recommended to assess the specimen's weight using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. "Head hair" specifically denotes scalp hair, while "body hair" encompasses all other forms (facial, axillary, etc.).

Process Overview

There are four primary steps in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab’s framework. It includes ensuring the sample was securely sealed and transported, allocating a unique LAN (Laboratory Accessioning Number), and finalizing any other data input not captured by an electronic chain of custody system.

Screening comprises a rapid preliminary examination for drug traces. While Screening offers a cost-efficient method to exclude drug use in most samples, a positive result necessitates validation to hold up in legal proceedings. Samples that are initially flagged positive during Screening require further confirmation.

If a sample shows a presumptive positive result during Screening, additional hair is extracted from the original sample for the Extraction phase. During this phase, drugs are isolated from hair at a significantly lower concentration than in other testing methods (e.g., urine or oral fluid), which is why hair drug testing is considered the most complex method to execute.

Positive screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with a presumptive positive outcome are cleansed before Confirmation as necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
