A hair sample of 1.5 inches, approximately 200 strands (about the thickness of a #2 pencil) near the scalp, yields 100mg of hair, which is optimal for initial screening and confirmation. For tests such as EtG, additional analysis, and/or more than 10 panels, 150mg is preferred. It's advisable to use a jeweler’s scale for weighing the sample. If scalp hair isn't available, an equivalent amount of body hair can be used. "Head hair" refers specifically to scalp hair, while "body hair" includes other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into the lab system, ensuring the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and performing any remaining data entry outside an electronic chain of custody.

Screening provides a preliminary check for drug presence. While this step efficiently rules out drug usage for most samples, any positive screen must be confirmed for court admissibility. Presumptively positive samples in Screening need secondary confirmation.

For samples that are presumptively positive during Screening, additional hair is extracted from the sample and processed for Extraction. At this stage, drugs are isolated from hair at concentrations lower than in other testing methods (e.g., urine or oral fluid), which explains the complexity of hair drug screening.

Confirmation is executed using GC/MS, GC/MS/MS, or LC/MS/MS for positive screening results. All samples presumed positive undergo necessary washing before confirmation. The entire process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
