A 1.5-inch segment of roughly 200 hair strands (similar to a #2 pencil's thickness) nearest the scalp will yield 100mg of hair, which is perfect for both screening and validation. For EtG add-ons or tests exceeding 10 panels, 150mg of the hair sample is suggested. It’s advised to measure the sample using a jeweler's scale. If scalp hair isn't available, an equivalent body hair sample can be taken. References to head hair pertain solely to scalp hair, whereas body hair encompasses all other types (such as facial or axillary hair).

Process Overview

The core four stages in lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is about integrating a sample into a lab's system. This process involves ensuring the sample was properly sealed and sent, assigning it a random LAN (Laboratory Accessioning Number), and doing any additional data input that's absent in an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. While cost-effective for dismissing drug use in most samples, a positive screen has to be confirmed to be used in court. Any samples with a presumptive positive in Screening need a subsequent confirmation.

If a sample is presumptive positive during Screening, more hair is extracted from the original specimen and set for Extraction. At this phase, drugs are extracted from hair at significantly lower levels compared to other techniques (such as urine or oral fluid), making hair drug testing the most challenging method to administer.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all presumptive positive samples are cleansed as required. The complete lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair label and the compliance with ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
