When taken near the scalp, hair offers a detection window of up to about three months for alcohol and other substances. Hair collection is straightforward, challenging to tamper with, and easy to transport.

Collecting a 1.5-inch segment of roughly 200 hair strands (approximately the size of a #2 pencil) nearest the scalp yields 100mg of hair, the preferred quantity for both screening and confirmation. For EtG, additional testing, and/or tests exceeding ten panels, 150mg of the sample is advised. It is recommended to use a jeweler’s scale to weigh the sample. If scalp hair cannot be obtained, a comparable quantity of body hair can be used. "Head hair" implies scalp hair only, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The laboratory process of analyzing a drug test encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where the sample enters the lab's system. This step involves ensuring the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data that is not available via an electronic chain of custody system.

Screening entails conducting a preliminary check for substances of abuse. Although this is a cost-efficient method to exclude drug use in most samples, a positive result from Screening requires Confirmation for legal admissibility. Any presumptive positives from Screening necessitate secondary verification.

For presumptive positives during Screening, additional hair is extracted from the original sample and readied for the Extraction step. In Extraction, substances are isolated from hair at lower concentrations than other methods (e.g., urine or saliva), making hair drug testing the most complex procedure to execute.

Confirmation of any positive Screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with presumed positive results are cleaned before Confirmation as required. Throughout, the lab process from Accessioning to Confirmation is reviewed under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
