Process Overview

The procedure of testing a sample in the lab involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with logging the sample into the lab’s system. This phase ensures that the sample was appropriately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and includes conducting any extra data entry not captured by an electronic chain of custody system.

Screening conducts an initial, rapid assessment for substance abuse. Screening is economical for excluding drug use in most samples. However, to be considered conclusive in a legal setting, any positive screen must be verified. Samples with presumptive positive results in the Screening phase require additional confirmation testing.

For samples showing presumptive positives during Screening, further hair is extracted from the original sample and prepared for Extraction. This stage focuses on extracting drugs from hair at concentrations lower than found in other methods (e.g., urine or saliva), making hair testing the most challenging method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS technologies handle the Confirmation of any positive screening outcomes. Samples that are presumptively positive undergo necessary washing before Confirmation. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
