For EtG, add-ons, and tests exceeding 10 panels, 150mg is suggested. We advise using a jeweler's scale to weigh samples. If scalp hair is not available, an equivalent amount of body hair may be used. By head hair, we mean scalp hair, whereas body hair includes other types (facial, axillary, etc.).

Process Overview

The primary phases in processing a drug test in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the sample’s entry into the lab’s processes. It guarantees the sample is sealed, properly shipped, assigns a LAN (Laboratory Accessioning Number) randomly, and concludes any additional data input absent from the electronic chain of custody system.

Screening is a preliminary rapid assessment for drug abuse. While economical for eliminating drug use in most samples, a positive result in Screening must be verified to stand in court legally. Samples with presumptive positives in Screening need confirmation with secondary tests.

For samples marked presumptive positive during Screening, more hair from the primary specimen undergoes Extraction. During this, drugs are extracted at much lower concentrations than in alternatives like urine or oral fluids, explaining why it's challenging to conduct hair drug screening.

The Confirmation step for any positive Screening is done through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before further confirmation if needed. The entire lab process, from Accessioning to Confirmation, adheres to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
