For EtG, supplementary tests, or panels exceeding 10, it is advisable to obtain 150mg of the hair. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair is not accessible, a similar quantity of body hair can be utilized. References to head hair imply scalp hair exclusively. Body hair denotes any other type of hair (such as facial, axillary, etc.).

Process Overview

There are four key stages in the lab processing of drug test samples: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary integration of a sample into a lab’s system. This step ensures the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and manages any additional data entry omitted by an electronic chain of custody system.

Screening provides an initial assessment for substances of abuse. Although Screening economically screens most samples for drug presence, any positive results should be validated to stand scrutiny in legal proceedings. Samples indicated as positive during Screening must undergo further Confirmatory analysis.

If a specimen tests positive during the Screening phase, additional hair is extracted from the collected sample and goes through the Extraction process. This stage retrieves drugs from hair at much more diluted concentrations compared to other methods (such as urine or saliva), making hair drug screening the most challenging to perform accurately.

Positive results from Screening receive Confirmation testing via GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptively positive sample is cleaned before Confirmation when necessary. The complete lab process from Accessioning to Confirmation is audited per CAP (College of American Pathologists) Hair guidelines and ISO/IEC 17025 accreditation standards.
