What it is, How it works

Hair analysis has emerged as a robust resource for unveiling drug and alcohol consumption. Hair strands encapsulate markers in their fibers, offering a comprehensive long-term history of alcohol and drug usage. When acquired near the scalp, hair can reveal drug and alcohol usage within a window of roughly three months. Collecting hair is easy, difficult to tamper with, and convenient to send.

A sample of 1.5 inches with approximately 200 hair strands (about the circumference of a #2 pencil) nearest to the scalp provides 100mg, the optimum amount for testing and validation. For EtG enhancements and tests exceeding 10 panels, 150mg is advisable. It’s suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't accessible, the same quantity of body hair is an alternative. By head hair, we mean scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the introduction of a sample into a laboratory’s records. This step involves ensuring the sample was properly sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and finalizing any additional data not included in an electronic chain of custody system.

Screening offers a preliminary rapid assessment for drugs. Although it’s an economical approach to discard drug usage for most samples, a positive screening necessitates confirmation for legal acceptability. Samples deemed presumptively positive in the Screening phase require further confirmation.

If deemed presumptively positive during Screening, more hair is extracted from the initial sample for Extraction. During Extraction, substances are isolated from hair at considerably lower levels compared to other techniques (e.g., urine or oral fluid), underscoring why hair drug testing is among the most challenging techniques.

For any positive screening outcomes, Confirmation is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive samples undergo washing prior to confirmation if needed. The full lab procedure from Accessioning to Confirmation undergoes scrutiny under the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

Advantages of hair drug testing:

  • Extended detection period: Hair drug evaluations can trace drug use up to 90 days, unlike urine tests with shorter detection durations.
  • Challenge to falsify: Cheating a hair drug test is difficult, leading to more reliable outcomes.
  • Offers historical data: It can display a drug usage pattern over time, not merely recent use.

Limitations:

  • Inability to detect recent use: Drugs become noticeable in hair after roughly 5-7 days.
  • Expense: Hair drug assessments tend to be costlier than other testing approaches.
  • Result variability: Elements like hair color and individual growth fluctuations can impact drug metabolite levels in hair.

Note: Despite being commonly termed “hair follicle tests”, the test evaluates the hair strand, not the follicle beneath the scalp
