A 1.5-inch sample consisting of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred standard for analysis and validation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We advocate weighing the specimen using a jeweler’s scale. If scalp hair cannot be collected, an equivalent quantity of body hair is acceptable. When referencing head hair, we specifically refer to scalp hair. Body hair includes all other hair varieties (facial, axillary, et cetera).

Process Overview

The laboratory drug test procedure encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a laboratory’s records. This step encompasses ensuring the sample’s proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entries absent from an electronic chain of custody system.

Screening involves an initial rapid examination for substances of abuse. Although Screening is a cost-effective method to eliminate drug presence in most samples, a positive screen demands confirmation for legal admissibility. Any sample presumptively positive in Screening requires further confirmation.

If a sample appears positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. Here, drugs are isolated from hair at a much lower concentration than in alternative methods (e.g., urine or oral fluid), making hair drug testing particularly challenging to perform.

Confirmation of any positive Screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
