What it is, How it works

When gathered near the scalp, hair can offer a detection timeframe of up to around three months for alcohol and other substances. Hair is straightforward to collect, relatively tough to tamper with, and easy to send.

A 1.5-inch segment of approximately 200 hair strands (similar to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect sample size for analysis and verification. For EtG, extra tests, and/or tests exceeding 10 panels, a specimen of 150mg is suggested. Using a jeweler’s scale to measure the specimen weight is recommended. If scalp hair cannot be obtained, an equivalent volume of body hair may be gathered. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The four critical steps in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a laboratory’s system. This process verifies that the sample was adequately sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not supplied by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. While Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be confirmed to be admissible in legal proceedings. Any samples with presumptive positivity in Screening necessitate a secondary confirmation.

If a sample shows presumptive positivity during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than other procedures (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is carried out through GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positivity are cleaned before confirmation if required. The entire laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
