What it is, How it works

Hair testing has emerged as a potent means for detecting alcohol and drug use. Hair offers a prolonged history of substance use by capturing biomarkers within the fibers of the developing hair strand. When collected near the scalp, hair can deliver an approximately three-month detection timeframe for both alcohol and drugs. Hair is straightforward to collect, relatively resistant to tampering, and simple to transport.

A sample measuring 1.5 inches with around 200 hair strands (equivalent to the diameter of a #2 pencil) nearest to the scalp will provide 100mg of hair, the ideal quantity for screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We recommend using a jeweler's scale for weighing the specimen. If scalp hair is not available, an equivalent amount of body hair can be obtained. When mentioning head hair, we are specifically referring to scalp hair. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab's system. This includes confirming that the sample was sealed and sent accurately, assigning a random LAN (Laboratory Accessioning Number), and performing any additional data entry not facilitated by an electronic chain of custody system.

Screening consists of an initial quick assessment for drugs of abuse. While Screening is a cost-effective method to rule out drug use in most samples, a positive screen necessitates confirmation for court admissibility. Any samples that are presumptively positive in Screening do require a secondary confirmation.

If a specimen tests positive in the Screening phase, additional hair is taken from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening presents the most challenging methodology to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleaned prior to confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
