For EtG, supplementary tests, and/or screenings beyond 10 panels, a 150mg sample is advised. It's recommended to measure the sample on a jeweler’s scale. If scalp hair is not present, an equivalent amount of body hair can be collected. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The lab processing of a drug test outcome involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample within a lab’s framework. This involves ensuring the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any further data input absent from an electronic chain of custody system.

Screening is an initial swift check for drug misuse. While Screening offers a cost-effective method to exclude drug use for most samples, any positive findings require confirmation to be valid in legal scenarios. Samples presumptively positive in Screening necessitate an additional confirmation.

If Screening suggests a presumptive positive, further hair is extracted from the initial sample for the Extraction stage. Here, drugs are extracted from hair at a far lower concentration compared to other techniques (such as urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Any positive screening outcome is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The full lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
