If scalp hair is not accessible, an equal quantity of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab's system. This step ensures the sample was appropriately sealed and transported, allocates a random LAN (Laboratory Accessioning Number), and finalizes any remaining data entry that an electronic chain of custody doesn't cover.

Screening performs an initial rapid assessment for drug misuse. Although Screening effectively eliminates drug usage for the bulk of samples, a positive result necessitates confirmation for legal validity. Presumptive positives in Screening require further verification.

In the event of a presumptive positive at the Screening phase, additional hair is extracted from the originating sample for Extraction preparation. This phase sees drugs removed from hair at significantly lower concentrations than alternate methodologies (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening findings is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed as needed before confirmation. The entire laboratory procedure—spanning Accessioning to Confirmation—is vetted under the CAP (College of American Pathologists) Hair designation alongside accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
