What it is, How it works

Hair maintains an extended record of alcohol and drugs by capturing biomarkers in the structure of the developing hair strand. When gathered next to the scalp, hair can reveal a detection timeframe of up to about a 3-month period for alcohol and other substances. Hair is straightforward to collect, somewhat hard to tamper with, and simple to transport.

A sample of 1.5 inches in length, consisting of roughly 200 hair strands (the equivalent thickness of a #2 pencil) nearest to the scalp, will yield 100mg of hair, which is the optimal sample for both screening and confirming. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. We suggest measuring the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent portion of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The lab processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the initial handling of a sample within a lab's system. This step includes verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not included by an electronic chain of custody system.

Screening involves an initial rapid check for drug abuse. Despite being a cost-effective method to rule out drug intake for most samples, a positive screen must be confirmed to hold up in court. Any samples that are presumptively positive during Screening need a secondary confirmation.

If a sample is deemed presumptively positive during the Screening phase, additional hair is drawn from the original sample and readied for Extraction. In this phase, drugs are extracted from hair, present at much lower concentrations than in other methods (ex. urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screen result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire lab process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
