What it is, How it works

Hair testing has emerged as an effective means for identifying the consumption of drugs and alcohol. Hair retains a prolonged record of alcohol and drug use by capturing biomarkers within the growing hair strand's fibers. When obtained near the scalp, hair can offer a detection period of up to around three months for alcohol and drugs. Hair samples are straightforward to gather, relatively tamper-resistant, and easy to transport.

A sample measuring 1.5 inches, consisting of roughly 200 hair strands (about the size of a #2 pencil) taken nearest to the scalp, provides 100mg of hair, which is the ideal quantity for screening and confirmation. For EtG, additional analyses, and/or those exceeding 10 panels, 150mg of hair is advised. We suggest weighing the sample on a jeweler’s scale. Should scalp hair be unavailable, an equivalent amount of body hair can be used. When referencing head hair, we mean scalp hair exclusively. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample in a laboratory's system. This step includes confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and performing any further data entry not covered by an electronic chain of custody system.

Screening includes a preliminary rapid assessment for drugs of abuse. Although Screening is an affordable method to exclude drug usage in most samples, a positive screening outcome necessitates confirmation for legal admissibility. Samples that have a presumptive positive in Screening require additional confirmation.

If a sample is potentially positive at the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluids), highlighting why hair drug screening poses the greatest challenge.

The confirmation of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned before confirmation as required. The complete lab process from Accessioning to Confirmation undergoes a review aligned with the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
