For EtG, additional tests, and/or screenings beyond 10 panels, 150mg of the sample is advisable. We propose weighing the sample on a jeweler’s scale. If scalp hair is not present, an equivalent amount of body hair may be taken. When mentioning head hair, we are referring solely to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four fundamental phases involved in the laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary handling of a specimen into a lab's system. This process involves validating that the sample was secured and transported correctly, allocating an arbitrary LAN (Laboratory Accessioning Number), and fulfilling any remaining data entry absent in an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. While Screening is an economical method to exclude drug consumption in most samples, a positive screen must be corroborated to be permissible in legal matters. Samples showing presumptive positives in Screening require a subsequent confirmation.

When a sample shows a presumptive positive in the Screening phase, more hair is extracted from the initial sample and arranged for Extraction. During this phase, drugs are removed from hair at a substantially lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug testing the most complex technique to implement.

Confirmation of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The complete laboratory protocol from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
