When gathered near the scalp, it offers up to a 3-month detection period for alcohol and drugs. Hair collection is straightforward, generally resistant to tampering, and simple to transport.

A 1.5-inch portion of around 200 hair strands (about the diameter of a #2 pencil) near the scalp provides 100mg of hair, the optimal quantity for both screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg is advised. Utilize a jeweler’s scale for precise weighing. In the absence of scalp hair, an equivalent quantity of body hair may be collected. When we mention head hair, it specifically refers to scalp hair only. Body hair encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the first stage of placing a sample into a lab's system. It includes ensuring the sample was efficiently sealed and transported, providing a random LAN (Laboratory Accessioning Number), and entering any additional data not documented by an electronic chain of custody system.

Screening involves a preliminary quick assessment for illicit drugs. While it's a cost-efficient method to rule out drug use for most samples, a positive screen must be confirmed for court acceptance. Any samples that test presumptively positive in Screening require further confirmation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are obtained from hair at much lower concentrations than other techniques (like urine or oral fluid), explaining why hair drug screening is among the most challenging methodologies.

Confirmation of any positively screened result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as necessary. The entire lab process from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
