A 1.5-inch sample comprising around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is optimal for screening and verification. For EtG tests, additional panels, or tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent amount of body hair can be used. When mentioning head hair, we are referring exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial integration of a sample into a lab’s system. It includes ensuring the sample was correctly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and performing any extra data entry not covered by an electronic custody chain system.

Screening is a preliminary evaluation for substances of abuse. Although it is a cost-efficient method to rule out drug use in most samples, a positive result must be confirmed to be valid in court. Samples that are presumptively positive during Screening require further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. Here, drugs are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to conduct.

Confirmation of any positive screenings is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed prior to confirmation if necessary. The comprehensive lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
