If scalp hair cannot be obtained, an equivalent mass of body hair can be used. "Head hair" specifically denotes scalp hair. "Body hair" encompasses all other types (such as facial or axillary hair).

Process Overview

There are four primary stages in lab processing for a drug test result: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially incorporated into the lab’s system. This involves ensuring the sample’s proper sealing and shipping, assigning a LAN (Laboratory Accessioning Number), and completing any additional data capture not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for substances of abuse. Though Screening is a cost-efficient method to disprove drug use in most cases, a positive result requires confirmation to be court-admissible. Samples that are presumptively positive in Screening necessitate subsequent confirmation.

For samples deemed presumptively positive at the Screening phase, further hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at notably lower concentrations than those in other samples types (such as urine or oral fluid), which makes hair drug screening a particularly challenging methodology.

Positively screened results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing before confirmation if necessary. The whole lab process from Accessioning through Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair standard and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
