What it is, How it works

Hair acts as a persistent record of alcohol and drug intake by embedding biomarkers within the hair strand’s fibers. When cut close to the scalp, hair can offer an approximate detection window of three months for alcohol and drugs. Collecting hair is straightforward, not easily tampered with, and convenient to transport.

A 1.5-inch hair sample comprising about 200 strands (roughly equivalent to a #2 pencil) from the scalp provides 100mg of hair, which is the perfect quantity for both screening and confirmation. For EtG, additional testing, and/or tests exceeding 10 panels, 150mg of the sample is preferable. It is advised to weigh the specimen using a jeweler's scale. If hair from the scalp is not available, the same quantity of body hair can be used. When we speak of head hair, we mean scalp hair specifically. Body hair encompasses all hair types excluding scalp hair, such as facial or axillary hair.

Process Overview

The key steps in the laboratory processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory system. This step includes confirming the sample’s proper sealing and shipment, generating a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry not included by an electronic chain of custody process.

Screening consists of an initial rapid assessment for drugs of abuse. While this is a cost-efficient means of dismissing drug usage for most samples, a confirmed positive screen is necessary for court acceptance. Any sample that screens presumptively positive needs further confirmation.

If a sample is identified as presumptively positive at the Screening phase, additional hair is taken from the primary specimen to prepare for Extraction. Confirmation of a positive screen result uses GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are cleansed before confirmation as required. The entire process, from Accessioning to Confirmation, is overseen under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
