What it is, How it works

Hair testing has emerged as a potent method for the detection of drug and alcohol consumption. When collected near the scalp, hair can offer up to roughly a 3-month detection period for alcohol and other drugs. Hair is easy to collect, relatively difficult to alter, and simple to ship.

A 1.5-inch section of about 200 strands of hair (approximately the size of a #2 pencil) nearest to the scalp will provide 100mg of hair, the perfect sample for testing. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We recommend using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent quantity of body hair can be collected. When discussing head hair, it refers exclusively to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages involved in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a specimen into a lab’s system. This process includes verifying that the specimen was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available via an electronic chain of custody system.

Screening involves an initial rapid check for drugs of abuse. Though Screening is a cost-effective solution to rule out drug usage on most specimens, a positive screen must be confirmed for court admissibility. Any samples showing a presumptive positive result in Screening require a secondary confirmation.

If a sample shows a presumptive positive result in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Confirmation of any positive screening is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
