A 1.5-inch segment consisting of around 200 hair strands (resembling the girth of a #2 pencil) nearest to the scalp yields 100mg of hair, the preferred quantity for analysis and validation. For EtG assessments, additional tests, and/or examinations beyond 10 panels, obtaining 150mg of the sample is advised. Utilizing a jeweler’s scale for measuring the sample is suggested. Should scalp hair be unobtainable, a comparable quantity of body hair can be used instead. The term head hair refers exclusively to scalp hair, while body hair encompasses all other types of hair (including facial, axillary, etc.).

Process Overview

The principal steps involved in processing a drug test in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Any confirmation of a positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation as required. The full lab procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair certification and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
