Hair evaluation has become acknowledged as a significant method for discovering drug and alcohol consumption. Hair provides an extensive history of alcohol and other substances as it traps biomarkers in the fibers of the developing hair strand. When sampled near the scalp, hair offers approximately a 3-month detection period for alcohol and other substances. Hair collection is straightforward, relatively challenging to tamper with, and easy to transport.
A 1.5-inch section of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the preferred quantity for testing and verification. For EtG, additional tests, and/or assessments exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler's scale to weigh the specimen. If scalp hair isn't accessible, a comparable quantity of body hair may be gathered. The term "head hair" refers specifically to scalp hair. "Body hair" encompasses all other types of hair (facial, axillary, etc.).
Process Overview
The lab processing of a drug test result encompasses four principal steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning pertains to the initial processing of a sample into a laboratory's system. This involves confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not supplied by an electronic chain of custody system.
Screening involves an initial rapid assessment for substances of misuse. While Screening is a cost-effective method to exclude drug use in the majority of samples, a positive screen must be confirmed to be valid in court. Any samples that preliminarily indicate positive in Screening require secondary confirmation.
If a sample shows a presumptive positive in the Screening stage, more hair is taken from the initial specimen and readied for Extraction. In Extraction, drugs are withdrawn from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.
Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleaned prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Note: Although often referred to as "hair follicle tests," the test assesses the hair strand rather than the hair follicle beneath the scalp.
