These tests are chosen for organizations enforcing strict anti-drug policies as they offer a 90-day detection window, collect under supervision, and minimize tampering risks. Despite these benefits, urine tests still provide precise detection of very recent drug use, specifically within a 5-day window.

What it is, How it works

Hair testing is recognized as a robust method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and other drugs by embedding biomarkers within the strands as they develop. When sampled near the scalp, hair can offer a detection period of up to three months for alcohol and various drugs. Hair collection is straightforward, challenging to tamper with, and easy to transport.

Collecting a 1.5-inch segment of about 200 hair strands (roughly the thickness of a #2 pencil) closest to the scalp yields 100mg, the optimal quantity for initial examination and verification. For EtG, additional testing, and/or analyses beyond 10 panels, 150mg is advisable. We suggest measuring the sample with a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair may be used. By head hair, we mean only scalp hair. Body hair encompasses all other types (face, armpit, etc.).

Process Overview

The laboratory process of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, integrating a sample into a lab's system. This process includes confirming the sample was appropriately sealed and sent, generating a random LAN (Laboratory Accessioning Number), and finalizing any supplemental data input absent from an electronic chain of custody system.

Screening carries out an initial swift assessment for drugs. Although Screening is an economical way to exclude drug usage for most samples, a positive must be validated for court acceptance. Any samples that preliminarily indicate positivity in Screening necessitate subsequent verification.

If a sample appears positive in Screening, additional hair is drawn from the initial sample and readied for Extraction. In this phase, drugs are retrieved from hair at much lower levels than by other methods (e.g. urine or saliva), which is why hair drug screening is the most complex technique to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all tentative positive samples are cleansed as required. The entire laboratory workflow, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair classification and conforms to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
