For EtG tests, additional panels, or tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to measure the specimen's weight. In situations where scalp hair is not available, an equivalent quantity of body hair is an alternative. When we mention head hair, it specifically refers to scalp hair. Body hair includes other hair types (such as facial or axillary hair).

Process Overview

The laboratory procedure for processing a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a sample into a lab’s system. This step verifies if the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any ancillary data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient approach to eliminate drug usage in most samples, a positive screening result must be validated to be legally permissible. Samples that return a presumptive positive from Screening require a subsequent confirmation.

Should a sample test presumptive positive during the Screening phase, additional hair is extracted from the original specimen for the Extraction phase. At this step, drugs are isolated from hair at much lower levels than alternative methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to perform.

Confirmation of any positive screen employs techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptive positive undergo washing before confirmation when necessary. The entire process, from Accessioning to Confirmation, is examined following both the CAP (College of American Pathologists) Hair designation and the standards set by ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
