Should scalp hair be unavailable, an equivalent body hair sample can be obtained. 'Head hair' denotes scalp hair exclusively, while 'body hair' encompasses other types like facial or axillary.

Process Overview

The principal stages in lab processing for a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial incorporation of a sample into a lab’s system. This step ensures the sample was sealed and sent properly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not covered by an electronic chain of custody.

Screening provides an early rapid test for drugs of abuse. Despite being an economical method to eliminate drug usage across most samples, a positive screen must be confirmed to uphold in legal contexts. Samples presumed positive in Screening need subsequent verification.

Should a sample appear positive during Screening, more hair is extracted from the original specimen and readied for Extraction. Here, drugs are isolated from hair at a significantly lower concentration compared to other methods (like urine or oral fluid), rendering hair drug screening the most intricate method to conduct.

Confirmation of any positive screening outcome is done through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleansed before confirmation as necessary. From Accessioning through to Confirmation, the whole laboratory procedure is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
