What it is, How it works

Hair retains a lengthy history of alcohol and drug use through biomarkers embedded within the fibers of the developing hair strand. When gathered near the scalp, hair can offer up to nearly a 3-month detection window for alcohol and other drugs. The collection of hair is straightforward, relatively tamper-resistant, and simple to transport.

A 1.5-inch segment containing roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal quantity for testing and verification. For EtG, additional tests, and/or analyses exceeding 10 panels, a specimen of 150mg is suggested. We advise using a jeweler’s scale to measure the specimen. If scalp hair is inaccessible, an equivalent volume of body hair can be collected. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves preparing a sample into a laboratory’s system. This encompasses confirming that the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not captured by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Screening is an economical means to discount drug usage in most samples; however, a positive screen must be confirmed to be legally accepted. Presumptively positive samples require subsequent verification.

For samples deemed presumptively positive during Screening, additional hair is extracted from the original specimen for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than through other methods (ex. urine or oral fluid), underscoring why hair drug screening is the most challenging methodology to carry out.

Confirmation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positives undergo washing, as needed, before confirmation. The entire laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
