Process Overview

The drug test laboratory procedure comprises four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample initially into a lab's system, which encompasses confirming the sample was appropriately sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data input that an electronic chain of custody might lack.

Screening provides an expedient preliminary assessment for drug abuse. While Screening offers a cost-efficient approach to dismiss drug use in most cases, any sample testing positive necessitates confirmation for court admissibility. Any presumptively positive samples in Screening must undergo additional confirmation.

If the Screening stage indicates a presumptive positive result, more hair is collected from the original sample for the Extraction process. This stage extracts drugs from hair at lower concentrations than other methods (e.g., urine or oral fluid), highlighting hair drug screening’s complexity.

Confirmation of any positive in Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptively positive samples are cleansed prior to confirmation as deemed necessary. The whole laboratory process, from Accessioning to Confirmation, is examined under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
