What it is, How it works

When hair is taken near the scalp, it offers a detection window of up to around three months for alcohol and drugs. A 1.5-inch segment of approximately 200 hairs (comparable to a #2 pencil's diameter) closest to the scalp yields 100mg of hair, which is the preferred sample amount for initial and confirmatory assessment. For EtG, additional options, and/or tests surpassing 10 panels, a 150mg specimen is suggested. Utilizing a jeweler’s scale for weighing the sample is advisable. If scalp hair cannot be obtained, an equivalent portion of body hair can be collected. In terms of head hair, it refers solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process in a drug test result involves four critical stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of logging a sample into a lab's system. This step verifies that the sample was adequately sealed and sent, generates a random LAN (Laboratory Accessioning Number), and finishes any extra data entry not provided via an electronic chain of custody system.

Screening acts as a preliminary quick check for substance misuse. Though Screening is a budget-friendly method to eliminate drug use in most samples, a positive screening needs confirmation to be viable for legal proceedings. Samples that test presumptively positive during Screening necessitate further confirmation.

When a specimen tests presumptively positive at the Screening stage, additional hair is taken from the original sample for Extraction. During this stage, drugs are pulled from hair at drastically lower concentrations than other methods (e.g., urine or oral fluids), explaining why hair drug screening presents more challenges.

Confirmation of any positive result from screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
