What it is, How it works

Hair analysis is now recognized as an effective means for detecting drug and alcohol consumption. Hair retains a prolonged record of alcohol and drug usage by enmeshing biomarkers within the fibers of the maturing hair strand. When hair is gathered near the scalp, it can offer nearly a 3-month timeframe for detecting alcohol and drugs. It is straightforward to collect hair, somewhat challenging to tamper with, and convenient to ship.

A sample of 1.5 inches, approximately 200 strands of hair (akin to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional testing, and/or panels exceeding 10, a 150mg sample is advisable. It's recommended to weigh the sample using a jeweler’s scale. In cases where scalp hair isn't available, an equivalent quantity of body hair can be taken. The term head hair refers strictly to scalp hair, while body hair encompasses all other types (like facial, axillary, etc.).

Process Overview

The lab procedure for a drug test result entails four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing within a lab’s system. This step verifies proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and wraps up any additional data entry absent in an electronic chain of custody setup.

Screening comprises an initial swift examination for drug use. Although Screening is cost-effective for negating drug usage in most samples, a positive result requires confirmation for legal admissibility. Presumptively positive samples during Screening necessitate secondary confirmation.

Should a sample appear presumptively positive during Screening, more hair is extracted from the initial specimen for preparation in the Extraction stage. In this phase, drugs are extracted from hair at much lower concentrations than other methods (such as urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as needed. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
