What it is, How it works

Hair testing is now recognized as an effective method for identifying drug and alcohol consumption. Hair offers a long-term record of alcohol and other drugs by encapsulating biomarkers in the strands of the growing hair. When gathered near the scalp, hair can offer a detection window of up to approximately 3 months for alcohol and other drugs. Hair is easy to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch sample consisting of about 200 strands of hair (roughly the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the preferred amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be used. When discussing head hair, we mean only scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step for processing a sample into a lab’s system. This step includes checking that the sample was properly sealed and transported, assigning a randomized LAN (Laboratory Accessioning Number), and finishing any extra data entry not supplied by an electronic chain of custody system.

Screening is a preliminary rapid check for substances of abuse. Although Screening is a cost-efficient method to eliminate drug use for most samples, a positive result must be confirmed to be considered valid in legal situations. Any samples that are presumed positive in Screening need a secondary confirmation.

When a sample is presumed positive during the Screening phase, additional hair is taken from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging method to perform.

Confirmation of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples are cleaned before confirmation if needed. The full laboratory process from Accessioning to Confirmation is verified under the oversight of both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
