A 1.5-inch section comprising roughly 200 hair strands (similar to a #2 pencil in size) closest to the scalp will yield 100mg of hair, the optimal sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg amount of the sample is advisable. It's suggested to measure the sample using a jeweler’s scale. Should scalp hair be unavailable, an equivalent quantity of body hair may be used. By head hair, we specifically mean scalp hair. Body hair indicates all other types (like facial or axillary hair).

Process Overview

The laboratory processing of a drug test result is composed of four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the primary steps of integrating a sample into a lab’s framework. This step involves validating that the sample was securely sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and performing any further data entry absent in an electronic chain of custody system.

Screening provides an initial brief examination for drugs of abuse. Though Screening is economical and rules out drug presence in most samples, a confirmed positive is necessary for court acceptability. Samples indicating a presumptive positive during Screening require additional confirmation.

If a sample tests presumptively positive in Screening, extra hair is extracted from the initial sample for the Extraction step. During Extraction, drugs are drawn from the hair at significantly lower levels compared to other methods (such as urine or oral fluid), leaving hair drug screening as the most complex method to conduct.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if required. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
