What it is, How it works

A 1.5-inch sample comprising approximately 200 hair strands (roughly akin to a #2 pencil) taken nearest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and confirmation tests. For EtG, supplementary tests, and/or panels exceeding 10, a 150mg specimen is suggested. We advise measuring the specimen with a jeweler's scale. In the absence of scalp hair, an equivalent amount of body hair may be assembled. By head hair, we refer exclusively to scalp hair, whereas body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The drug test result laboratory process primarily entails four steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the lab's system, ensuring the sample was sealed and shipped appropriately, generating a random LAN (Laboratory Accessioning Number), and completing additional data entry absent from the electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. While Screening serves as a budget-friendly way to exclude drug usage in the majority of cases, any positive screen necessitates confirmation to be admitted in legal contexts. Samples exhibiting presumptive positivity in Screening require secondary verification.

When a sample shows presumptive positivity during the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), rendering hair drug screening as the most intricate methodology to execute.

Confirmation of any positive screenings is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before confirmation. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
